AI assessment for nursing competency
Rubric-graded, human signed-off. Not a quiz generator.

Scorafy is the assessment-feedback layer for nursing courses. It reads a student's open-ended answers, clinical scenario responses, reflective journals and uploaded video, audio or files, then scores them against your own rubric with cited evidence pulled straight from the submission. A qualified assessor reviews every result and signs off before it is recorded, so no judgement is made by software alone. It is not an LMS or a student-management system - it sits alongside the tools you already use and handles the grading and feedback.
What you assess for nursing competency
- Aseptic technique and infection control reasoning in a written or video-recorded clinical scenario
- Medication calculation and the rights of administration, including how the student justifies dose checks
- Patient assessment and clinical handover (for example an ISBAR or SBAR handover) for completeness and clinical accuracy
- Reflective practice and recognition of scope of practice, including when to escalate to a registered nurse
- Documentation against the relevant standards, including consent and privacy of patient information
A worked rubric criterion
You define the criteria and levels. Scorafy grades each answer against them, with the evidence.
Criterion
Performs a clinical handover (ISBAR) that communicates the patient situation safely and completely
Why a quiz won't cut it
Nursing is judgement under pressure, not recall. A multiple-choice quiz can confirm a student knows the five rights of medication administration, but it cannot show whether they would actually check them, recognise a deteriorating patient, or escalate at the right moment. Those decisions live in how a student explains their reasoning and what they do in a scenario. Scorafy grades that open-ended evidence against your rubric so the judgement is assessed, not just the fact.
Written, video, audio or file
Video and audio matter for nursing because so much of the competency is in the doing and the communicating. A student can upload a recorded simulation of a patient assessment or a spoken clinical handover, and Scorafy reads the actual performance against your rubric rather than relying on a written summary of it. Uploaded files such as care plans or medication charts are assessed alongside the recording.
A qualified person signs off
Scorafy drafts the scoring against your rubric and references the evidence. A qualified assessor reviews every result, overrides anything, and finalises it. The AI score is kept alongside the human-final score as an audit trail, so no decision is solely automated and every result can be explained.
Compliance and defensibility
Nursing evidence often contains health information, so handling it carefully is not optional. Scorafy is built compliance-first under the GDPR and the EU AI Act. Assessment is high-stakes, so a qualified assessor reviews and signs off every result - there is no solely-automated decision about whether a student is competent. Every score keeps an audit trail showing the evidence cited and the assessor who confirmed it, which is what you need to defend a result at validation or audit.
Frequently asked questions
Can Scorafy assess a recorded clinical simulation, not just written work?
Yes. A student can upload a video of a simulated patient assessment or a recorded ISBAR handover. Scorafy reads the recording against your rubric and cites the moments that support each judgement, then the assessor reviews and signs off.
Does Scorafy make the final decision on whether a nursing student is competent?
No. Scorafy scores the evidence against your rubric and shows its reasoning, but a qualified assessor reviews every result and signs off before it is recorded. There is no solely-automated competency decision.
How does Scorafy handle patient information in student submissions?
It is built compliance-first under the GDPR and EU AI Act, with an audit trail on every result. You set the rubric and your assessors control what is recorded, so health information in evidence is handled within your existing privacy obligations.
Is Scorafy an LMS for our nursing course?
No. Scorafy is the assessment-feedback layer. It grades open-ended evidence and gives feedback against your rubric, and it works alongside whatever LMS or student-management system you already run.
